What happened

Employee was using a foot pedal operated press brake. Employee put a piece of metal in the press brake and stepped on the foot pedal to bend the part. Employee 's foot was not fully removed from pedal. Employee leaned forward to remove the part their foot put pressure on the pedal and activated the press brake. Employee 's fingers became caught between the tool and dye. Employee broke right middle finger.
