Before the incident

It was a normal day. The vinyl went out of the skate. Mr. [REDACTED] left the safe zone in an attempt to slide the skate back into position. Mr. [REDACTED] did not listen to his coworkers as he left the safe zone. The skate came off and he then fell.

What happened

Mr. [REDACTED] went outside of the safe zone without proper PPE and he subsequently fell.
