On [REDACTED] [REDACTED] was walking near the copier machine in the [REDACTED] office area. She tripped on a Cintas carpet mat and fell. When she fell she hit her arm and shoulder on the table next to the copier and then fell to the ground and landed on her knee. The mat was not lying flat on the floor which caused her to trip on it.
