Before the incident

EE was engaging the elevator lift controls in attempting to free a stuck cheese cart that was stuck at the top of the lift.

What happened

The bracket connecting the fork to the chain sheered a bolt allowing the bucket and fork to free fall down and EE tried to jump away and hit his right hip leg on the lift bucket support
