Before the incident

On [REDACTED] employee notified me that she tweaked her back while breaking down a glass pallet. This requires the employee to remove the cardboard from the pallet to put into the cardboard bailer. The employee was struggling to remove the stapled cardboard from the pallet and pulled up on the cardboard with extra force causing discomfort in her lower back. The employee at that time said that she was fine and would take it easy and would check in with me.; On [REDACTED] the employee informed me that over the weekend working on her car her back is still bothering her and she was having troubles sleeping.
