What happened

On [REDACTED] Ms. [REDACTED] injured herself on while providing care to a resident. She turned the resident and immediately felt pain in her left shoulder. Ms. [REDACTED] reported this incident to our former Director of Nursing and former unit manager both did not report or investigate the incident. On [REDACTED] Ms. [REDACTED] came to my office to get a form and inform me of her injury . Told her to complete the incident form and I would report it to workmens comp. She went to St. John Providence and DMC for treatment. This the reason for late reporting I was unaware .
