What happened

Employee stepped down and caught the edge of the 4x4 wood blocking that the rod bundles rest on their way down to the ground. Employee was stepping on to the lower frame of the pipe rack to push the asphalt pipe forward to fill the inlet of inbound side of [REDACTED]. when they were stepping down i was told another individual was talking to them which may have caused a distraction also.
