What happened

Employee was working under the truck changing the oil pan basket. The employee states that he had the proper jack stands under the truck and was using those while working under the truck. In the video you cannot see any jack stands being used while he was under the truck. Employee states that the jack stands slipped off the axle and the axle fell onto his chest while under the truck.
