Before the incident

While a box of fished goods was being pushed through Bag Machine 12 's auto packer the shark fin pusher got lodged on the 2nd shark fin flap folder while it was in the process of returning to the home position. A 2nd box of finished goods was moving into position to be processed through the auto packer when it also got jammed due to the shark fin pusher not being able to return to the home position. When the EE cleared the jams the shark fin pusher broke free and returned to the home position. As the shark fin pusher returned to the home position it struck the EE in the face.

What happened
