Packing Cardboard into Hopper of truck using his foot to push cardboard into truck

What happened

Put last piece of cardboard in hopper and holding onto safety bar pulled levers to begin packing process. While holding safety bar he stepped down backwards onto edge of truck his hand slipped off bar and right leg slid off edge of hopper down between truck and dock
