Forklift Operator incrementally raised the forks higher to allow distance to remove the anchor bolt and on the third bump the Tipper slid off the forks.

Forklift Operator incrementally raised the forks higher to allow distance to remove the anchor bolt and on the third bump the Tipper slid off the forks hitting the IE 's right pinky finger and causing a fracture and laceration.
