At approximately [REDACTED] [REDACTED] reported a possible hernia sustained while moving stacks at the [REDACTED] location When asked if he required medical attention he declined and stated he was okay noting he would monitor his condition and provide updates However at [REDACTED] [REDACTED] called again to inform that he will be seeking medical attention related to the incident I advised him to submit a statement along with the hospital information for the facility he will be visiting Hes sending the information via text with all management copied on it Information was entered into Risk but it hasnt populated in [REDACTED] Hes currently at St Elizabeth Hospital ([REDACTED]) [REDACTED] reported that he requires surgery to repair the hernia
