Before the incident

Employee was on a scissor lift with another employee painting over a sign they removed.

What happened

The other employee controlling the lift moved to the side with the lift cage up still and went at an angle causing the lift to go off the walkway. Which caused the lift to tip over and fall with both employees in it.
