Before the incident

Installing 3rd floor wood beams. Standing in scissor lift setting wood beans crane used to hoist beams into place. Hammer used to adjust & position beam.

What happened

Employee was in a scissor lift setting wood beams. The beam was bound up on steel knife blade as employee was hitting a shim block used to position the beam the beam freed up and dropped down approx. 3 inches pinching his fingers between beam & knife blade.
