Before the incident
EE was directing forklift driver who was change out an empty pallet of chemicals with a new pallet of drums. EE was standing on the block wall near the skid.
What happened
The forklift driver lowered its forks onto the edge of the chemical containment skid causing it to snap back and eject liquid splashing EE in the face and chest with water caustic. EE was not wearing chemical-specific PPE only safety glasses.
