Before the incident
[REDACTED] was standing at the rear of tandem dump truck IBM [REDACTED] helping with removal of the tailgate to install a brine bucket in the bed. He was getting ready to grab a support chain to hook up the tailgate to the loader bucket.
What happened
On [REDACTED] around [REDACTED] [REDACTED] was assisting crew with removal of tailgate from tandem aspalt truck IBM [REDACTED] for installation of a brine tank into the bed. While at the rear of the truck he was attempting to grab a chain to help hook up onto the loader bucket for removal. The tailgate became unattached from the truck and f[REDACTED]l strking another [REDACTED] in the head body legs. [REDACTED] was knocked down and his legs became pinned by the tailgate. Several [REDACTED] 's came to his aid and pushed the tailgate off of him.
