Loading pallets into the trailer.

Worker was loading pallets into the trailer using the forklift. The stack was too high he proceeded into the truck and the stack of pallets shifted due to hitting the dock door. He exited the forklift and climbed the stack of pallets in an attempt to physically push the load lost his footing fell off the stack of pallets onto the floor on his ankle and the pallet fell on him.
