I came back from break at [REDACTED]. At [REDACTED] pt. was on the side of the bed. He needed to use the bathroom and I handed him the urinal. After using the bathroom he gave me the urinal back I sat it down and was helping get pt. back into bed. The pt. sat down and started pushing me off of him. I tried to hold both arms and the pt. kicked me on the side of my head. Injury to neck and head
