[REDACTED] was working in [REDACTED] Residence [REDACTED] checking in with Caregiver’s and MedTech’s to make sure they didn 't need any assistance and helping residents with their walkers after dinner.

What happened

[REDACTED] noticed [REDACTED] (resident) was wandering and was trying to redirect [REDACTED]; they were having a nice conversation about what other activities she could do in an attempt to get her back to her residence [REDACTED] suggested [REDACTED] could go for a walk. As [REDACTED] turned to her right to start walking towards [REDACTED] [REDACTED] slapped [REDACTED] open handed on the left side of her face. In response [REDACTED] put her right arm up to block [REDACTED] from slapping her again then [REDACTED] grabbed [REDACTED] 's right arm and started digging her fingernails into her forearm while twisting it.
