A co-worker and a member of maintenance was moving a dead electric pallet jack on a forklift to the maintenance shop. [REDACTED] was walking by the area.

The pallet jack needed to be centered on the forklift forks and the employee adjusted the forks on the pallet jack. After doing this the wheel on the pallet jack retracted and nipped the tip of his left index finger.
