Stated that she was in the corner of the store by the rear door sitting on a step ladder while bending over stamping bottles.

Fell back onto the frame of the door and then to the floor as the step ladder suddenly collapsed.

Further stated that at the time of the incident she did not feel any pain. Now her lower back is sore.

She was instructed not to sit on any ladder. The step ladder was examined and one of the joints of the ladder is bent. The ladder was removed from Provisions.
