Before the incident

Employee was having his annual hearing evaluation performed.

What happened

Annual hearing evaluation showed an OHSA recordable hearing loss in the right ear. Afterwards the employee ended up having a family emergency and needed to take time off of work for an extended period of time. Since he was gone we were unable to have his hearing retested to confirm the hearing loss. Therefore we are reporting it on the OSHA log even though it was never confirmed.
