What happened

Employee was manually lifting the dock ramp. Dock leveler was stuck. Employee used his foot to hold the handle on the floor and grabbed a hook to lift the leveler. As employee pulled the hook it slipped off and caused employee to lose his balance. He fell on his back and left side of his hip.
