What happened
Electrician was retrieving a light fixture from the FMCC warehouse. After collecting the fixture; Mr. [REDACTED] proceeded to load it into the rear of a rented bucket truck.He initially placed the light fixture on the rear passenger side of the vehicle. He then climbed onto the truck using the rear steps and stepped onto the rear platform. Instead of using the designated side entry point; Mr. [REDACTED] attempted to climb over the side of the truck by stepping onto the top of the toolbox; while attempting to position himself and retrieve the light fixture employee lost his footing. As a result he fell to the ground along with the light fixture; sustaining a fracture to his right wrist.
