Before the incident

I was checking to make sure a blade was seated properly on a knife handle it was not slipped off and poked my fingertip. I was in OR #7 doing a total hip with Dr. The blade came off as he made incision I put back on using a needle holder I was checking it it came off again. It was on my right hand pointer finger.
