A maintenance employee was performing a [REDACTED] on the Forge Lube Vac [REDACTED] The employee removed the belt cover to complete a step of the PM The belt cover was not replaced prior to starting the machine on the next step of the PM The employee noticed a substance spraying from the machine and pressed the stop button The employees right hand came in contact with the belt resulting in the right middle finger to be caught in between the belt and pulley
