Before the incident

On Bag Machine #627 EE was cleaning a printing plate on the print cylinder. EE noticed a piece of cardboard wedged between the print cylinder & impression cylinder. EE attempted to pull the stuck cardboard with their left hand. Once the EE pulled the cardboard in an upward motion the print cylinder & impression cylinder moved pinching the tip of the EE’s left middle & ring fingers.

What happened

On Bag Machine #627 EE was cleaning a printing plate on the print cylinder. EE noticed a piece of cardboard wedged between the print cylinder & impression cylinder. EE attempted to pull the stuck cardboard with their left hand. Once the EE pulled the cardboard in an upward motion the print cylinder & impression cylinder moved pinching the tip of the EE’s left middle & ring fingers causing an avulsion to both fingers & a fracture to the ring fingertip.

Left middle & ring finger avulsion & left ring fingertip fracture. Note: EE referred to Texas Health Plano Presbyterian Emergency Department after initial encounter at Concentra Medical Center.
