What happened

Employee was washing wet concrete residue off a pipe mold set. The employee used a rolling step platform to gain access to the top of the mold set. The water hose he was using was laying on the steps. While walking down the steps the employee tripped on the water hose causing them to stumble. As the employee was attempting to regain his balance he hit his leg on part of the pipe pro pipe machine.
