What happened

Was attempting to insert IV into patient right fore arm. Seem to have hit a valve and the needle would not advance. Had another nurse attempt to flush the catheter while I attempted to float the catheter past the valve. The catheter slipped out of the vein while the other nurse was flushing causing a bloody mist into this nurses face.
