Before the incident

The driver used a 48 ' x 48 ' wood pallet to climb onto the back of the truck to hand unload door onto an H cart being raised by the Home Depot receiver

After the driver finished hand unloading the door from on top of the bed of the truck the driver took a step toward the back end of the truck lost his footing and fell about 51 ' onto the asphalt.
