Before the incident

Employee was loosening a lock on a shaft with an allen wrench.

What happened

Employee used right hand to push allen wrench down to remove the screw on the lock. Allen wrench slipped and employee 's back of right hand went into keyway on the shaft causing a cut on the knuckles and back of hand.

Cut to back of right hand. Cut to tendon.
