Before the incident

Attempting to de-escalate an agitated patient who was having a psychotic episode while another RN intramuscularly injected patient with medication.

What happened

After the other RN injected the patient she handed the syringe to the employee without knowing that the safety mechanism (retracting) was not engaged. The EE was stuck with the dirty needle on the palmar surface of his right index finger.
