Replacing cylinder on Chip N Saw spline remover

What happened

Employee was using a hammer and punch to drive the cylinder clevis pin out of the clevis. When the clevis pin came out the spline remover arm dropped onto the punch on the inside of the clevis. This in turn pinched the employee 's left pinky finger between the punch and clevis pin hole on the outside of the clevis.
