Before the incident

[REDACTED] was grabbing LVL from the rack on north side of building. He slipped on the ice and fell on his left shoulder. He laid there for a few minutes to catch his breath got up and grabbed the LVL and loaded it on the truck.

What happened

On [REDACTED] [REDACTED] came in late in the day and said he fell on the ice while pulling some LVL. He was alright and I reminded him of wearing his spikey 's. He said I know I forgot.He finished his day with no further mention. On [REDACTED] [REDACTED] came into work and said his shoulder hurt. He said he iced it last night and again [REDACTED] and took some tylenol.
