What happened

loss date on original paperwork see documents. He lifted a piece of lumber and it hurt him a little bit but he didnt think it was a big deal. He mentioned it to [REDACTED] and [REDACTED]. He had it checked by his private doctor and they sent him to do therapy. It got better but later at work he has encountered the same pain. He went back to his doctor and this time they diagnosed him with a herniated disc undocumented. Timeline from original injury was approx 3 weeks after therapy to the re-injury and new diagnosis. We will update info as [REDACTED] provides doctors paperwork.
