What happened

Employee was pulling cable with co-worker. He was standing on ladder feeding cable into the adjacent compartment when he told his co-worker to stop pulling so he could put the cable tag on the cable. The co-worker did not hear and pulled the slack through the collar causing employee to suddenly turn on ladder and bump his head on a cableway hanger in the overhead.
