What happened

Employee placed left hand on cutter guard to reach up and grab a strand of pasta being pushed between the 2 dies on A-Line. A gap in the guarding that he placed his hand on allowed his left index finger to extend down into the cutter zone as he extended forward and reached upward resulting in an avulsion to the left index fingertip.
