What happened

While the employee was sweeping another employee called for her. When she started to walk toward the employee she tripped on the top of the bolt that is in the floor holding the pillars in place. She lost her balance and fell toward another pillar striking her forehead on the pillar. Employee refused to go to the clinic at the time of the incident. She went [REDACTED] the [REDACTED]. The clinic completed an exam and then sent her to the ER for further tests.
