Before the incident

The employee was lining up a metal sheet on the 1000 burn table.

What happened

The employee tried to move the metal sheet north but the corner of it was stuck in the slat tray. This caused the opposite end of the material to come up and rotate clockwise. The material then came free and swung into the employee. This drove them back into a small hopper where the injuries happened.
