Employee was checking the delivery hose and realized that something was wrong he was going back to his truck to shut down the delivery process and correct the problem.

While walking back to his truck the driver tripped and fell at the same time the delivery hose came out of the tank and sprayed product on the tank ground employee and his truck trailer.
