What happened

[REDACTED] (Injured Person) was operating a standup forklift to retrieve his 3rd pallet of Mississippi Lime (Calcium Hydroxide) for use at the wet blend bag dumping station. While lifting the pallet of Calcium Hydroxide (40 bags 50lbs each) from a 3rd rack [REDACTED] lost control of the load and [REDACTED] bags fell off the pallet directly below the rack. This caused approximately [REDACTED] bags to open creating a cloud of Calcium Hydroxide and a significant amount of material spilled on the lower racks and warehouse floor. The [REDACTED] began cleaning up the spill lead and relief operator were notified of the event [REDACTED] to [REDACTED]. [REDACTED] stopping initial cleaning activities [REDACTED] to [REDACTED] after multiple requests by team members. [REDACTED] took a shower change uniform boots and donned chemical PPE to completed the spill clean–up.
