Before the incident

Mechanics were repairing a fixed ladder at Hassayampa pumping station. This work required the bottom support bracket that is fixed to the floor by anchor to be removed. This required grinding the ladder base.

What happened

To facilitate the grinding near the floor the mechanic was positioned on their knees to perform the grinding. This was discussed as opposed to laying on the floor. The grinder Kicked back and came in contact with the knees of the mechanic.
