What happened

S Phase II operator reported that he injured his left ring finger after it made contact with the idler roll attached to the side of the duo box O The investigation determined that at the time of the incident the associate was using his left hand with a rag in it to wipe lint off the side of the [REDACTED] while the position was running The associate was unsure if the idler caught his glove or the rag but his finger was pulled against the idler roll injuring his finger The associate should not have been cleaning around spinning equipment The position should be down before cleaning A The associate was sent to see the plant nurse for treatment A meeting was held with the all Phase II associates to remind them to not place their hands or any other body part around spinning equipment The position should be off and all rolls at a complete stop before wiping down the equipment P A incident alert was sent to all WM associates to make them aware of the incident and to advise not to clean equipm
