What happened

TM works at the stacking hoist and states that he began to feel pain in his right heel. He left work at [REDACTED] and once he had returned home he left to be seen at the emergency room at [REDACTED] hospital in [REDACTED]. He informed me that he had a torn achilles this wasn 't the case based on the report from his follow up appointment. (He can 't confirm what caused the pain there was no hard copy incident report from the second shift team and it was reported at [REDACTED] directly from [REDACTED] on [REDACTED]).
