shift lead was calling for the attention of the employee who was grinding on the OD of cylinder bevel. The employee went to stand up when he bumped his nose into a coupon that was attached to the cylinder. in the action of bumping the cylinder he also hit his knee on a 3 foot tall bundle of wood dunnage that was stage nearby.
