Before the incident

[REDACTED] was reaching across the conveyor with can grabber tool to pick up tipped over cans.

What happened

The conveyer was running while he was doing this. His right arm contacted the conveyer at that point it grabbed his arm and hand and sucked it under the dead plate. His right arm and hand were stuck between the dead plate and conveyer. At that point he started yelling [REDACTED] heard him and ran upstairs and E stopped the conveyer. When I got there the conveyer war stopped and me and a few guys were able to pull the conveyer backwards enough to release his arm and hand from under the dead plate. (Information provided by supervisor [REDACTED]).
