Before the incident
Employee was working at Composite at #1 Plasma Bench. The employee had successfully cut 2 plates and 4 mults on #1 Plasma Bench before the incident occurred. On the third plate the employee had cut a “2-out-of-1 finish plate” which results in an additional scrap piece being generated in the center of the two plates. The employee was removing the scrap (i.e.. side and end) from the plates and realized the center scrap had fallen between two support rails. At this point the employee’s inexperience on the job led to a less than optimal decision of removing the two finish plates prior to attempting to remove the scrap. The employee walked out onto the plasma table on the rails and attempted to hookup the piece of scrap from the far side of the bench using two stirrup hooks. During our investigation with the employee the employee stated he removed the two plate sides (i.e. work surfaces) because he had to gain access to center cut scrap piece that had fallen between the rails. Lifting device was not properly attached to scrap piece and employee and investigating team believe it got caught under support rail gusset resulting in the hook dislodging.
