Before the incident

Walking across the parking lot [REDACTED] [REDACTED] I noticed spots that appeared wet but were icey so I walked slow and shuffled When I got to the sidewalk by the ENT door it appeared dry Halfway to the ENT door I slipped and fell It was icey Once I got up and in the building I took Ice [REDACTED] went back outside and put some on the spots on the sidewalk to try and prevent others from falling
