What happened

While carrying a 4 'x8 ' x 1 2 sheet of plywood and walking on the roof trusses he slipped causing him to loose his balance and fall his hard hat fell off and his head hit a sheet of plywood that was already set on the trusses. He was wearing fall protection he did not fall to the ground.
