EE WAS WORKING SORT ON [REDACTED] AND REPORTED ON [REDACTED] THAT HE HAD 40LB BOX THROWN ON HAND DURING SORT. HE TOLD AM MANAGER THAT HE DID NOT WANT TO WAIT AND SEE ONE OF OUR DOCTORS NOR DID HE WANT TO HAVE INJURY ENTERED THAT HE WOULD HAVE HIS INSURANCE BILLED. HE LEFT AND WENT TO HOSPITAL WHERE THEY SAW HIM. A BILL WAS SENT TO FEDEX BY HIM.
